For a conventional spine approach, the retractors retain the back muscles that have been peeled off of their attachments to the vertebra.  Using an operating microscope to enhance visibility, the surgeon  can use a nerve root retractor to manually move the nerve roots out of the way so that a rongeur can reach into the disk space to remove part of the nucleus pulposus.
